Background: The classical symptom specificity hypothesis (Blatt, 1974) links obsessional symptoms to autonomous interpersonal behavior. Inconsistent findings from cross-sectional group studies on symptom specificity have previously been associated with several conceptual and methodological limitations intrinsic to nomothetic research. Previous empirical case research reported ambivalences between autonomous and dependent interpersonal behavior in obsessional pathology. Aim and Method: The present ‘theory-building’ case study specifically aims at further refinement of the classical symptom specificity hypothesis by testing specific operationalizations within an empirical single case (...) design. Consensual Qualitative Research for Case studies is used to quantitatively and qualitatively describe the longitudinal, clinical interplay between obsessional symptoms and interpersonal dynamics throughout the process of a psychodynamic therapy. Interpersonal functioning is assessed by means of the Core Conflictual Relationship Theme method. The classical symptom specificity hypothesis is contrasted to an alternative hypothesis of interpersonal ambivalences as determinants of obsessional symptoms. Results: Close associations between obsessions and interpersonal dynamics are observed. Findings predominantly affirm the ambivalence hypothesis rather than the classical symptom specificity hypothesis. Psychodynamic interventions focusing on interpersonal conflicts are related to symptomatic changes. Aim: Understanding the effects of psychotherapy is a crucial concern for both research and clinical practice, especially when outcome tends to be negative. Yet, while outcome is predominantly evaluated by means of quantitative pre-post outcome questionnaires, it remains unclear what this actually means for patients in their daily lives. To explore this meaning, it is imperative to combine treatment evaluation with quantitative and qualitative outcome measures. This study investigates the phenomenon of non-improvement in psychotherapy, by complementing quantitative pre-post outcome scores (...) that indicate no reliable change in depression symptoms with a qualitative inquiry of patients’ perspectives. Methods: The study took place in the context of a Randomized Controlled Trial evaluating time-limited psychodynamic and cognitive behavioural therapy for major depression. A mixed methods study was conducted including patients’ pre-post outcome scores on the BDI-II-NL and post treatment Client Change Interviews. Nineteen patients whose data showed no reliable change in depression symptoms were selected. A grounded theory analysis was conducted on the transcripts of patients’ interviews. Findings: From the patients’ perspective, non-improvement can be understood as being stuck between knowing versus doing, resulting in a stalemate. Positive changes (mental stability, personal strength and insight) were stimulated by therapy offering moments of self-reflection and guidance, the benevolent therapist approach and the context as important motivations. Remaining issues (ambition to change but inability to do so) were attributed to the therapy hitting its limits, patients’ resistance and impossibility and the context as a source of distress. “No change” in outcome scores therefore seems to involve a “partial change” when considering the patients’ perspectives. Conclusion: The study shows the value of integrating qualitative first-person analyses into standard quantitative outcome evaluation and particularly for understanding the phenomenon of non-improvement. It argues for more multi-method and multi-perspective research to gain a better understanding of (negative) outcome and treatment effects. Implications for both research and practice are discussed. In this paper I will discuss one of the soteriological models Thomas Aquinas outlines in his Summa Theologiae, namely ‘sacrifice’. This is only one of several, but not mutually exclusive, ways in which Thomas interprets our salvation in Christ. I will briefly list the other models before focussing in more depth on sacrifice by considering some objections against it. I will continue by outlining Thomas's theology of sacrifice in its own right and explain its connections with the Eucharist. By way (...) of conclusion I will return to the modern criticism raised. Medieval theology, in all its diversity, was radically theo-centric, Trinitarian, Scriptural and sacramental. It also operated with a profound view of human understanding. In a post-modern climate, in which the modern views on 'autonomous reason' are increasingly being questioned, it may prove fruitful to re-engage with pre-modern thinkers who, obviously, did not share our modern and post-modern presuppositions. Their different perspective does not antiquate their thought, as some of the 'cultured despisers' of medieval thought might imagine. On the contrary, rather (...) than rendering their views obsolete it makes them profoundly challenging and enriching for theology today. This book is more than a survey of key medieval thinkers ; it is an invitation to think along with major theologians and explore how their thought can deeply challenge some of today's modern and post-modern key assumptions. Broadly drawing on the writings of Thomas Aquinas, this article is a systematic-theological (rather than historical-theological) engagement with the theme of providence and divine causality. It aims to dispel some modern misunderstandings of these topics by highlighting how pre-modern approaches differ from today's perspective. It does so by arguing, firstly, that Thomas, given his teleological focus, construes divine causality not so much as efficient causality but rather in terms of final causality. I will also make the point that Thomas's calling (...) God a ‘universal cause’ should not be construed in terms of omni-causality, as if God predetermines every event (be it necessarily or contingently). In the final part of this contribution, I make some observations on the arbitrariness of afflictions and the connection with the gratuitousness of charity within the providential ordering. (shrink)

This contribution examines two related points in relation to Aquinas’s understanding of contemplation, which is a sorely neglected topic in scholarship. First, after having outlined that the final act of contemplation culminates in an intellective, simple apprehension of the truth, I will examine how this act relates to the three operations of the intellect (grasping of quiddity, judgement, and reasoning) Aquinas identifies in a number of places. Second, I argue that his view of contemplation as simple insight is significantly indebted (...) to Neoplatonic sources; therefore, we must pay attention to the way he introduces Neoplatonic elements into his Aristotelian framework. I conclude this contribution by suggesting some reasons—of a theological nature—why Aquinas would have been drawn towards a non-discursive or “intuitive” notion of contemplation. (shrink)

Developing countries face numerous barriers to conducting effective and efficient ethics reviews of international collaborative research. In addition to potentially overlooking important scientific and ethical considerations, inadequate or insufficiently trained ethics committees may insist on unwarranted changes to protocols that can impair a study's scientific or ethical validity. Moreover, poorly functioning review systems can impose substantial delays on the commencement of research, which needlessly undermine the development of new interventions for urgent medical needs. In response to these concerns, the Drugs (...) for Neglected Diseases Initiative, an independent nonprofit organization founded by a coalition of public sector and international organizations, developed a mechanism to facilitate more effective and efficient host country ethics review for a study of the use of fexinidazole for the treatment of late stage African Trypanosomiasis. The project involved the implementation of a novel ‘pre-review’ process of ethical oversight, conducted by an ad hoc committee of ethics committee representatives from African and European countries, in collaboration with internationally recognized scientific experts. Team-Based Learning is a comprehensive approach to using groups purposefully and effectively. Because of its focus on decision making, it is well suited to helping students learn to do philosophy and not simply talk about it. Much like the “flipped classroom” approach, it is structured so that students are held responsible for “covering content” through the reading outside of class so that class meeting times can be spent practicing philosophical decisions, allowing for frequent feedback from the professor. This chapter discusses (...) how TBL works in Philosophy, the elements of a TBL course including activity design, and how TBL avoids the known problems of group work. This article reports a study of how leading U.S. business schools incorporate one important dimension of corporate citizenship—corporate involvement in community economic development (CI/CED)—in their curricula and programs. Corporate citizenship, or social responsibility, is shown to have several important and unexpected locations in business education. In addition, the authors develop a rationale forwhy and howspecific topics such as CI/CED as well as the general topic of corporate citizenship are appropriate for business school attention.

Saunders & van Brakel set out to review color science research and to topple the belief that color-vision neurophysiology sets strong deterministic constraints on the cognitive processing of color. Although their skeptism and mission are worthwhile, they fail to give proper treatment to (1) findings that dramatically support some positions they aim to tear down, (2) existing research that anticipates criticisms presented in their target article, and (3) the progress made in the area toward understanding the phenomenon. At the very (...) minimum these oversights weaken the credibility of their arguments and leave the reader to wonder why their discussion ignores what is clearly omitted. (shrink)

It remains unclear whether such genetic variants are associated with disease aggressiveness. The NCI-SPORE Genetics Working Group retrospectively collected clinicopathologic information and genotype data for 36 SNPs which at the time had been validated to be associated with PC risk from 25,674 cases with PC. Cases were grouped according to race, Gleason score and aggressiveness. Statistical analyses were used to compare the frequency (...) of the SNPs between different disease cohorts. After adjusting for multiple testing, only PC-risk SNP rs2735839 was significantly and inversely associated with aggressive and high-grade disease in European men. Similar associations with aggressive and high-grade disease were documented in African-American subjects. The G allele of rs2735839 was associated with disease aggressiveness even at low PSA levels in both European and African-American men. Our results provide further support that a PC-risk SNP rs2735839 near the KLK3 gene on chromosome 19q13 may be associated with aggressive and high-grade PC. Future prospectively designed, case-case GWAS are needed to identify additional SNPs associated with PC aggressiveness. (shrink)

Despite the burgeoning literature on professionalism in other health professions, psychology lags behind in the level of attention given to this core competency. In this article, we review definitions from other health professions and how they address professionalism. Next, we review how this competency evolved within health service psychology (HSP), and we propose a definition. We offer an approach for assessing professionalism within HSP. Consideration is given to strategies and methods for providing effective education and training in this multifaceted competency. (...) Increasingly, professionalism has been recognized as a core competency for health service professionals and is the domain in which vexing competence problems are observed in trainees. We begin by describing manifestations of problems of professionalism in accord with the values that fall within the rubric of this multifaceted construct. We provide an approach for evaluating problems of professionalism and discuss intervention for trainees with mild, moderate, or severe problems in this domain. We propose implications for training focused on enhancing the (...) culture of programs; bolstering the education, guidance, and mentoring provided related to professionalism; and encouraging best practices for addressing trainees with problems of professionalism. We conclude by sharing ideas about defining professionalism, identifying problems of professionalism, strengthening our approach to assessing professionalism and intervening when problems are evident, developing strategies for preventing professionalism problems, and ensuring that psychologists take seriously their responsibility to address professionalism concerns with colleagues. System-level change is crucial for solving society's most pressing problems. However, individual-level interventions may be useful for creating behavioral change before system-level change is in place and for increasing necessary public support for system-level solutions. Participating in individual-level solutions may increase support for system-level solutions – especially if the individual-level solutions are internalized as part of one's social identity.

BackgroundRandomized controlled trials (RCTs) are often complex and expensive to perform. Less than one third achieve planned recruitment targets, follow-up can be labor-intensive, and many have limited real-world generalizability. Designs for RCTs conducted using cohorts and routinely collected health data, including registries, electronic health records, and administrative databases, have been proposed to address these challenges and are being rapidly adopted. These designs, however, are relatively recent innovations, and published RCT reports often do not describe important aspects of their methodology in (...) a standardized way. Our objective is to extend the Consolidated Standards of Reporting Trials (CONSORT) statement with a consensus-driven reporting guideline for RCTs using cohorts and routinely collected health data.MethodsThe development of this CONSORT extension will consist of five phases. Phase 1 (completed) consisted of the project launch, including fundraising, the establishment of a research team, and development of a conceptual framework. In phase 2, a systematic review will be performed to identify publications (1) that describe methods or reporting considerations for RCTs conducted using cohorts and routinely collected health data or (2) that are protocols or report results from such RCTs. An initial “long list” of possible modifications to CONSORT checklist items and possible new items for the reporting guideline will be generated based on the Strengthening the Reporting of Observational Studies in Epidemiology (STROBE) and The REporting of studies Conducted using Observational Routinely-collected health Data (RECORD) statements. Additional possible modifications and new items will be identified based on the results of the systematic review. Phase 3 will consist of a three-round Delphi exercise with methods and content experts to evaluate the “long list” and generate a “short list” of key items. In phase 4, these items will serve as the basis for an in-person consensus meeting to finalize a core set of items to be included in the reporting guideline and checklist. Phase 5 will involve drafting the checklist and elaboration-explanation documents, and dissemination and implementation of the guideline.DiscussionDevelopment of this CONSORT extension will contribute to more transparent reporting of RCTs conducted using cohorts and routinely collected health data. Sowohl die Semantik als auch ein Kalkül des natürlichen Schließens werden jeweils detailliert diskutiert. In projects concerning big data, ethical questions need to be answered during the design process. In this paper the Value Sensitive Design method is applied in the context of data-driven health services aimed at disease prevention. It shows how Value Sensitive Design, with the use of a moral dialogue and an ethical matrix, can support the identification and operationalization of moral values that are at stake in the design of such services. Rational agents base their actions on information from observation, inference, introspection, or other sources. But this information comes in different kinds, and it is usually handled by different logical mechanisms. We discuss how to integrate external ‘updating information’ and internal ‘elucidating information’ into one system of dynamic epistemic logic, by distinguishing two basic informational actions: ‘bare seeing’ versus ‘conscious realization’.

ABSTRACT Over the past few decades the concept of (human) dignity has deeply pervaded medical ethics. Appeals to dignity, however, are often unclear. As a result some prefer to eliminate the concept from medical ethics, whereas others try to render it useful in this context. We think that appeals to dignity in medical ethics can be clarified by considering the concept from an historical perspective. Firstly, on the basis of historical texts we propose a framework for defining the concept in (...) medical debates. The framework shows that dignity can occur in a relational, an unconditional, a subjective and a Kantian form. Interestingly, all forms relate to one concept since they have four features in common: dignity refers, in a restricted sense, to the ‘special status of human beings’; it is based on essential human characteristics; the subject of dignity should live up to it; and it is a vulnerable concept, it can be lost or violated. We argue that being explicit about the meaning of dignity will prevent dignity from becoming a conversation‐stopper in moral debate. Secondly, an historical perspective on dignity shows that it is not yet time to dispose of dignity in medical ethics. At least Kantian and relational dignity can be made useful in medical ethics. Constructive empiricism is indeed set squarely within a common sense realism that was foreign to much of the empiricist tradition. But I do not see this common sense realism, which I take myself to share with many scientific realists, as harboring or leading to scientific realism. That is in part because of the way I separate the opposition between empiricist and realist understanding of science from other issues that divide us in epistemology. This discussion brought to light our quite different (...) conceptions of what is at issue between empiricists and realists in this area. After a response to McMullin's critique, however, I will be especially concerned to respond to his challenging proposal for a shift in the debate over realism concerning the sciences. (shrink)

When is an argument to be called one-sided? When is putting forward such an argument fallacious? How can we develop a model for critical discussion, such that a fallaciously one-sided argument corresponds to a violation of a discussion rule? These issues are dealt with within ‘the limits of the dialogue model of argument’ by specifying a type of persuasion dialogue in which an arguer can offer complex arguments to anticipate particular responses by a critic.

Van Vugt, van der Velde, and collaborators show how cognitive architectures can implement verbal theories of psychiatric problems. They show how one theory of depressive rumination can be implemented in the ACT‐R cognitive architecture by changing the contents of its simulated memory. These manipulations of memory habits lead the model to show impairments in a sustained attention task‐‐a plausible impairment given that people who suffer from depression have concentration complaints.

In 1922, many representatives of the Russian Intelligentsia, including many philosophers, were exiled from the young soviet state. Many left with the so-called Philosophy Steamer (Chamberlain in The philosophy steamer: Lenin and the exile of the intelligensia (2006) Atlantic Books). The exiled philosophers tried to go on with their previous professional lives in cities as Prague, Berlin and Paris. The St. Serge Orthodox Theological Institute in Paris, founded by, among others, Sergei Bulgakov (1871–1944), became the new center of Russian religious (...) philosophy and theology in Europe. Soon, however, the community of immigrant Russian religious philosophers and theologians was divided by conflicting opinions, and fell apart in various brotherhoods and movements. An important conflict was the so-called Sophia controversy: the Brotherhood of St. Photius, which included Vladimir Lossky (1903–1958), as well as the famous spokesman of the Neopatristic Synthesis, Fr. Georges Florovsky (1893–1979), attacked the Brotherhood of St. Sophia, which included the above mentioned Bulgakov. His Sophiology, or study of Sophia, Divine Wisdom, was accused of heresy on the instigation of Florovsky and Lossky. From this Sophia controversy, the Neopatristic Synthesis emerged as the dominant school of Russian Orthodox Theology, whereas Sophiology fell practically into oblivion. This article will attempt to answer the question whether there is still room for a re-valuation of the sophiological stance in Orthodox theology through a survey of recent literature on the subject. The article will conclude the affirmative: namely, that the controversy is shown to be the result of a conflict between generations and the necessity to empower their Orthodox identity in emigration, rather than the result of intrinsic philosophical or theological differences. This first volume of the _Adages_ in the Amsterdam edition of the Latin texts of Erasmus gives a general introduction to the _Adages_ in German, as well as a critical edition of the Latin text of the first half of the first thousand _Adages_.

This paper analyzes the collaboration between healthcare professionals and algorithms in making decisions within the realm of public healthcare. By extending the concept of ‘tinkering’ from previous research conducted by philosopher Mol (Care in practice. On tinkering in clinics, homes and farms Verlag, Amsterdam, 2010) and anthropologist Pols (Health Care Anal 18: 374–388, 2009), who highlighted the improvisational and adaptive practices of healthcare professionals, this paper reveals that in the context of digitalizing healthcare, both professionals and algorithms engage in what (...) I call ‘collaborative tinkering’ as they navigate the intricate and unpredictable nature of healthcare situations together. The paper draws upon an idea that is increasingly common in academic literature, namely that healthcare professionals and the algorithms they use can form a hybrid decision-making entity, challenging the conventional notion of agency and intelligence as being exclusively confined to individual humans or machines. Drawing upon an international, ethnographic study conducted in different hospitals around the world, the paper describes empirically how humans and algorithms come to decisions together, making explicit how, in the practice of daily work, agency and intelligence are distributed among a range of actors, including humans, technologies, knowledge resources, and the spaces where they interact. The concept of collaborative tinkering helps to make explicit how both healthcare professionals and algorithms engage in adaptive improvisation. This exploration not only enriches the understanding of collaborative dynamics between humans and AI but also problematizes the individualistic conception of AI that still exists in regulatory frameworks. By introducing empirical specificity through ethnographic insights and employing an anthropological perspective, the paper calls for a critical reassessment of current ethical and policy frameworks governing human–AI collaboration in healthcare, thereby illuminating direct implications for the future of AI ethics in medical practice. Conflict zone entrepreneurs—local entrepreneurs running small businesses in conflict settings—have paradoxical impacts on stability: holding the ability both to foster peace but also to enhance conflict. Prior scholarly work has been unable to explain this divergence, as existing entrepreneurial indicators do not account for fundamental peacebuilding elements. In response, the article consolidates divergent fields of study, applies paradox theory to analyze underlying tensions in the field, and reframes entrepreneurship through a peacebuilding lens based on intergroup inclusivity and value-creating business practices. Using several cases, this article shows how entrepreneurial activity in conflict zones can foster sustainable peace, limited peace, or conflict, arguing that entrepreneurship based on value-creating and inclusive practices can result in pro-peace outcomes. The article offers policy recommendations for local governments and the humanitarian sector seeking to use entrepreneurship in conflict zones to promote peace. (shrink)

In his text ‘The modern development of the foundations of mathematics in the light of philosophy’ from around 1961, Gödel announces a turn to Husserl's phenomenology to find the foundations of mathematics. In Gödel's archive there are two draft letters that shed some further light on the exact strategy that he formulated for himself in the early 1960s. Transcriptions of these letters are presented, together with some comments.

Given a cofinal cardinal function $h\in {}^{\kappa }\kappa $ for $\kappa $ inaccessible, we consider the dominating h-localisation number, that is, the least cardinality of a dominating set of h-slaloms such that every $\kappa $ -real is localised by a slalom in the dominating set. It was proved in [3] that the dominating localisation numbers can be consistently different for two functions h (the identity function and the power function). We will construct a $\kappa ^+$ -sized family of functions h (...) and their corresponding localisation numbers, and use a ${\leq }\kappa $ -supported product of a cofinality-preserving forcing to prove that any simultaneous assignment of these localisation numbers to cardinals above $\kappa $ is consistent.
